The defect rate is related to internal processes
After measuring the defect rate and comparing to the standard, I must analyze the followings
1. Raw material
Did we change the supplier? Did we change the material? Did we stick to the quality control at reception of the orders?
2. Equipment
What is the maintenance calendar and what is contained in the last maintenance report? Is there any defect in the equipment of production or some equipment may need to be replaced?
Did we add a new equipment or change a process?
3. Employees
What is the average “lifespan” of the labor in the production team? How many (percentage) of them are newly appointed?
If there is a training program, what is the consistency of the production labor?
When last did we change the efficiency standard? What is the trend of the labor rate?
If there is any complain registry, is there any “pattern” revealed like health condition?
Corrections to be taken:
Depending on the answers to the questions above, the corrective measures must be planned.
If the issue is a stress on the labor because of a high standard of efficiency or laxity in quality control at reception, it can be easily addressed.
If the issue is a need of new equipment or training, the correction may take some time. It must be part of the next target.
